It is important to describe each field on your form so we can create the PDF to your specifications. Please be sure to describe all the fields on the form per the specifications below, and list them in the order they appear on the form. You can describe the fields in the email. Name this form to name that represents the form you will be submitting. For example, if your you are submitting is EFT Auth than name this form ST_EFT Auth. When completed, email this form and the form to be converted to [email protected].
Tell us about the fields on your form. Fields are the input boxes on your PDF which are filled in by the salon or customer. Most standard forms will usually have 3 fields. A Printed Name, Signature and Date fields.
