Only use this page if you have signed up for our PDF eSigner.
It is important to describe each field on your form so we can create the PDF to your specifications. Please be sure to describe all the fields on the form per the specifications below, and list them in the order they appear on the form. You can describe the fields in the email. If you will be sending more than one form in the email, please be sure the descriptions are grouped with form name. When ready, please email the form(s) to [email protected].
Your Forms.
Tell us about the fields on your form. Fields are the input boxes on your PDF which are filled in by the salon or customer. Most standard forms will usually have 3 fields. A Printed Name, Signature and Date fields.
Complete this PDF for each form to submit. Using this form helps us collect all required information. After filling it out, download and save the file with a name that reflects the form you are submitting (e.g., ST_EFT Auth for an EFT Auth from). Email both this completed form and the form to be converted to [email protected].
